EMD Serono remains committed to maximizing access to Serostim^®.

The Serostim^® Copay Assistance Program will decrease or potentially eliminate any out-of-pocket costs. Guaranteed up to $200 a month, for 6 months of treatment, for all eligible HIV-associated wasting patients receiving a new Serostim^® prescription.

• Can be used at all Serostim^® Secured Distribution Pharmacies

How it Works

1. Physician diagnoses and prescribes Serostim^®
2. Physician or health care team member hands the patient a Serostim^® Copay Assistance Card
3. Patient uses the card at pharmacy for automatic savings

* Note: If physician or health care team does not provide a card, SeroCare^SM can provide one if it processes the script.

1. No activation required by physician or patient
2. Mail in rebate is available if patient forgets or loses card
3. Patient has 12 months to utilize 6 uses
4. Potential savings of up to $1,200

Program Requirements

• Patient must have a prescription drug benefit that covers Serostim^®
• Patients must have a new or current Serostim^® prescription
• Card can be used 6 times in a 12 month period, no more than once every 21 days
• Card expires 12 months after activation (first use)
• Patients may not use card if patients receive drug benefits from Medicaid, a Medicare drug benefit plan, or other federal or state programs (includes state Medicaid assistance program and/or pharmaceutical patient assistance programs). Additionally, patients may not use card if their private insurance, HMO, or other health or pharmacy benefit program paid for the entire Serostim^® prescription
• Void in any state where prohibited by law, taxed, or otherwise restricted. Not valid in Massachusetts
• Patients may not combine this card with any other rebate, coupon, free trial, or similar offer. One card per person
• EMD Serono reserved the right to rescind, revoke, or amend this offer at any time without notice

Most patients will be eligible for the Serostim^® Copay Assistance Program.^*

With the Serostim^® Copay Assistance Program, 94% of privately insured patients will have their entire copay covered. The remaining 6% will have up to $200 of their copay covered.
